Decades-old question: Is antibacterial soap safe?

WASHINGTON (AP) ? It's a chemical that's been in U.S. households for more than 40 years, from the body wash in your bathroom shower to the knives on your kitchen counter to the bedding in your baby's basinet.

But federal health regulators are just now deciding whether triclosan ? the germ-killing ingredient found in an estimated 75 percent of antibacterial liquid soaps and body washes sold in the U.S. ? is ineffective, or worse, harmful.

The U.S. Food and Drug Administration is planning to deliver a review this year of whether triclosan is safe. The ruling, which will determine whether triclosan continues to be used in household cleaners, could have implications for a $1 billion industry that includes hundreds of antibacterial products from toothpaste to toys.

The agency's review comes amid growing pressure from lawmakers, consumer advocates and others who are concerned about the safety of triclosan. Recent studies of triclosan in animals have led scientists to worry that it could increase the risk of infertility, early puberty and other hormone-related problems in humans.

"To me it looks like the risks outweigh any benefit associated with these products right now," said Allison Aiello, professor at the University of Michigan's School of Public Health. "At this point, it's just looking like a superfluous chemical."

The concerns over triclosan offer a sobering glimpse at a little-known fact: Many chemicals used in everyday household products have never been formally approved by U.S. health regulators. That's because many germ-killing chemicals were developed decades ago before there were laws requiring scientific review of cleaning ingredients.

The controversy also highlights how long it can take the federal government to review the safety of such chemicals. It's not uncommon for the process to drag on for years, since regulators must review volumes of research and take comments from the public on each draft.

In the case of triclosan, Congress passed a law in 1972 requiring that the FDA set guidelines for dozens of common antibacterial chemicals found in over-the-counter soaps and scrubs. The guidelines function like a cookbook for manufacturers, detailing which chemicals can be used in what products, and in what amounts.

In 1978, the FDA published its first tentative guidelines for chemicals used in liquid hand soaps and washes. The draft stated that triclosan was "not generally recognized as safe and effective," because regulators could not find enough scientific research demonstrating its safety and effectiveness.

The FDA published several drafts of the guidelines over the years, but the agency never finalized the results. So, companies have not had to remove triclosan from their products.

Meanwhile, the agency did approve triclosan for use in Colgate's Total toothpaste in 1997, after Colgate-Palmolive Co. submitted data showing that the ingredient helped fight gingivitis.

Then, last summer, the FDA said its review of triclosan would be complete by late 2012. That target date then slipped to February, which has also come and gone. But pressure on the agency from outside critics didn't let up.

In March, a federal appeals court said a lawsuit by the nonprofit Natural Resources Defense Council aimed at forcing the FDA to complete its review could move forward. A three-judge panel reinstated the 2010 lawsuit, which had been tossed out by a lower court, saying the nonprofit group presented evidence that triclosan could potentially be dangerous.

Now, four decades after it was charged with reviewing triclosan, the FDA is planning to complete its review. FDA spokeswoman Stephanie Yao said evaluating triclosan and other antibacterial agents is "one of the highest priorities" for the agency, but did not offer an explanation for the delay.

The FDA's website currently states that "the agency does not have evidence that triclosan in antibacterial soaps and body washes provides any benefit over washing with regular soap and water."

The American Cleaning Institute, a cleaning products trade organization, says it has provided reams of data to FDA showing that triclosan is both safe and effective.

"Triclosan is one of the most reviewed and researched ingredients used in consumer and health care products," says Brian Sansoni, a spokesman for the group, whose members include Colgate-Palmolive and Henkel Consumer Goods Inc., maker of Dial soap.

While it can take years for the government to make rules, members of Congress say there is little precedent for the FDA's four-decade review of triclosan.

"When FDA first started evaluating the rules governing triclosan's use, Richard Nixon was still president," said Rep. Edward Markey, D-Mass, who asked the FDA to take a closer look at triclosan in 2010 after the European Union banned the chemical from products that come into contact with food.

"Science has evolved, and so should FDA's regulations guiding the use of this chemical in consumer products," he says.

U.S. scientists agree that the FDA's review is overdue. The Endocrine Society, a group of doctors and scientists who specialize in the hormone system, flagged triclosan four years ago as an ingredient that alters levels of thyroid hormones and reproductive hormones like testosterone and estrogen.

"I think the FDA is behind the curve," said Dr. Andrea Gore of the University of Texas at Austin, who was the lead author of the Endocrine Society's statement on hormone disrupting chemicals. "At what point do you draw a line and say we need to take this out of products that are being applied to our skin? What is enough evidence?"

Some Americans are shocked that the FDA has taken so long. Mallory Smith is troubled to learn that the government has never confirmed the safety of antibacterial soap's key ingredient.

Smith, who works for the federal government, says she keeps antibacterial soap in the kitchen to clean her hands after she's handled raw meat.

"As a regular consumer I rely on the government to identify products that are safe for me to use," Smith said. "If something is brought to their attention, they should look into it, and ban the chemical if necessary."

Others are less surprised by the government's multi-decade review. "It sounds like a typical government agency to me: totally unproductive," said David Fisher, who sells restaurant equipment in Arizona.

Ironically, triclosan first became widely used because it was considered safer than an older antibacterial ingredient, hexachlorophene. That chemical was banned from household items in 1972 after FDA scientists discovered that toxic levels could be absorbed through the skin. Several infant deaths in France were connected to baby powder that contained unsafe levels of the chemical, due to a manufacturing error.

Triclosan was initially used in hospitals in the 1970s as a scrub for surgeons preparing to perform an operation. It was also used to coat the surfaces of catheters, stitches and other surgical instruments.

Beginning in the 1990s, triclosan began making its way into hundreds of antibacterial consumer goods, ranging from soap to socks to lunchboxes. The growth has in part been fueled by Americans who believe that antibacterial ingredients provide an added level of protection against germs.

As the use of triclosan has expanded, more scientists have questioned its effectiveness. In 2007, researchers at the University of Michigan and other universities compiled data from 30 studies looking at the use of antibacterial soaps. The results showed soaps with triclosan were no more effective at preventing illness or reducing bacteria on the hands than plain soap.

Other studies have shown that longer hand-washing improves results far more than adding antibacterial ingredients. The Centers for Disease Control recommends washing hands at least 20 seconds. The CDC also recommends using hand sanitizer ? most of which use alcohol or ethanol to kill germs, not chemicals like triclosan ? if soap and water are not available.

Troclosan's safety also has become a growing concern in recent years. To date, nearly all of the research on triclosan's health impact comes from animal studies ?which are not necessarily applicable to humans ? but the findings still have researchers concerned.

A 2009 study by scientists at the Environmental Protection Agency showed that triclosan decreases levels of testosterone and sperm production in male rats. Female rats exposed to triclosan showed signs of early puberty and altered levels of estrogen and thyroid hormones.

And 2010 study by University of Florida researchers found that triclosan interfered with the transfer of estrogen to growing fetuses in pregnant sheep. Estrogen is important in both male and female development because it promotes growth of organs like the lungs and liver.

Sansoni, the soap and detergent industry spokesman, says those animal studies can't be applied to humans and "make exaggerated claims about the damaging effects" of triclosan.

But safety concerns over triclosan don't just involve rats and other animals. Some experts argue that routine use of antibacterial chemicals like triclosan is contributing to a surge in drug-resistant germs, or superbugs, that are immune to antibiotics. Few studies have attempted to track antibiotic resistance tied to Triclosan in the real world. But laboratory studies have shown that antibiotic-resistant strains of E. coli and other bacteria can grow in cultures with high levels of triclosan.

As a result of the growing concerns, some leading medical societies, hospitals and companies have abandoned the chemical.

Kaiser Permanente pulled triclosan from its 37 hospitals across the country in 2010, switching to traditional soaps and alcohol-based hand sanitizers. Kathy Gerwig, Kaiser Permanente's vice president for workplace safety, said the hospital chain decided to phase out triclosan as part of its "precautionary approach" to safety issues.

"If there is credible evidence that a product we're using might have some disadvantages from a health or environmental standpoint, then it's our obligation to look for a safer alternative," Gerwig said.

Johnson & Johnson has pledged to remove triclosan from all of its adult products by the end of 2015. The company says none of its baby products currently contain the ingredient.

"We want people to have complete peace of mind when they use our products," Susan Nettesheim, vice president of product stewardship, said when the company made the announcement last summer.

American Idol: So close, yet so far

On Thursday, American Idol bid farewell to one of their remaining four finalists, denying them the coveted opportunity to become hometown heroes for a day. Which Idol is going home to tears rather than fanfare?

Ryan was right when he said that this elimination, right before hometown visits is arguably the most difficult of the season. The awareness of this impending difficulty was etched on the faces of every hopeful as they were forced to endure another long, drawn out episode featuring past contestants and current performers hoping to tap into Idol's diminishing fan base before it flatlines all together.

On Thursday, the roster included past winner David Cook who (I'm pleased to report) was looking markedly less creepy than he did during Idol's notorious season 7, also known as the David versus David season. In addition to reducing fine lines and creep-factor, David Cook's Idol win also helped him to purchase a nice little house in Nashville, complete with three dogs and a yard.

Also on hand to take us on a trip down memory lane was Season 4's elimination shocker, Constantine Maroulis, who has the distinct pleasure of being one of the least attractive man to ever make me swoon. There is just something intoxicating about the heady blend of Bohemian Rhapsody and long flowing locks, which is why Dana Carvey's roll as Garth in Wayne's World forever solidified his role as a Hollywood Sex Symbol. Unlike Dana, Constantine went on to do great things after his iconic performance of Bohemian Rhapsody, such as earning himself a Tony nomination for his roll in the Broadway hit, Rock of Ages.

Joining such Idol elites was recording artist known simply by his first name, will.i.am. will.i.am dropped his last name, a.plagiarist, when he became an artist, fearing that it might prevent other artists from collaborating with him, at least knowingly. Harry Connick, Jr. also returned on Thursday for another moment in the Idol spotlight, this time performing a song from his new album. Harry really should have quit while he was ahead and ended on Wednesday's high note.?

Speaking of ending on a high note, finally, after four guests, two hideously painted Ford Fusions and 14,673 plugs for the movie, The Great Gatsby, Ryan dimmed the lights. As the numbers of contestants dwindle, it becomes more difficult to create anticipation with the results. No more easing into them throughout the course of the show. No more hopefuls herded into misleading groups on opposite sides of the stage. No, with just four left, the elimination is a perfunctory job saved for the last minutes of the show, the only real pleasure, a sadistic one that comes after Phil Phillips's sweet voice lulls the fallen Idol into a depressed delirium, when Ryan hands the loser the microphone to sing so we can all take bets on how many versus she'll make it through without falling apart.

This week, as predicted, that poor sacrificial lamb was the judges' favorite Amber Holcomb, whose swan song was punctuated by rogue sobs until finally she?succumbed?to the sad reality that she was so close, yet so tragically far from earning a hometown visit. Chin up Amber, it's clear that Nicki is jonesing for her own little Bieber and thinks you might just fit the bill.

UCSF scientist says new approach could 'democratize' viral surveillance

The tick-borne Lone Star virus has been conclusively identified as part of a family of other tick-borne viruses called bunyaviruses, which often cause fever, respiratory problems and bleeding, according to new research led by scientists at UC San Francisco (UCSF).

What made the work especially promising, said principal investigator Charles Chiu, MD, PhD, was the speed at which the virus was definitively identified. The team used a new approach to gene sequencing that enabled them to completely reconstruct the virus' previously unknown genome in less than 24 hours significantly faster than conventional sequencing techniques, which can take days to weeks.

The technique, called ultra-rapid deep sequencing, combines deep sequencing an emerging technology that reconstructs an entire DNA sequence from a tiny snippet of DNA with advanced computational techniques and algorithms developed in the laboratories of Chiu and his research collaborators.

Chiu, an assistant professor of laboratory medicine at UCSF and director of the UCSF-Abbott Viral Diagnostics and Discovery Center, reported the results in a paper published in PLOS ONE on April 29. It can be found online at http://www.plosone.org/.

The team found that the Lone Star virus, which is carried by the Lone Star tick, Amblyomma americanum, is related to a group of human pathogens including Severe Fever with Thrombocytopenia Syndrome Virus, which infected hundreds of farmers in China between 2008 and 2010; Bhanja virus, initially found in India; Palma virus, found in Portugal; and Heartland virus, an illness recently reported among farmers in Missouri.

"We did not show that Lone Star virus causes disease in humans," Chiu cautioned, "although the laboratory and sequencing data suggest that this is a distinct possibility."

He said the work may prove to be significant in light of the fact that nearly all emerging diseases discovered over the past two decades have originated in animals. While the causes of many human infectious diseases have been "pretty well characterized," he said, researchers have "only touched the tip of the iceberg" with respect to pathogens that have the potential to pass from animals to humans.

Chiu pointed to a number of serious and unexpected animal-to-human disease transmissions over the last 10 years, including SARS in 2003, the H1N1 influenza in 2009, and the current outbreak of H7N9 avian influenza, which already has resulted in more than 20 deaths in China.

"Nature is continually throwing us curveballs," Chiu said. "We will likely always be faced with the threat of novel outbreak viruses originating in animals or insects. It will be extremely important to identify and characterize those viruses as quickly as possible to get a head start on the development of diagnostic assays for surveillance and drugs, or vaccines for treatment before they have a chance to really spread."

In such circumstances, ultra-rapid deep sequencing will be "extremely useful," he said. "By the time SARS was identified and sequenced using conventional methods, more than a week of time had been lost. That kind of delay could be quite risky in a virus that spreads rapidly in human populations."

Chiu and his team plan to introduce a graphical user interface that will allow small laboratories to analyze and access ultra-rapid, deep-sequencing data through cloud computing over the Internet, even though they do not have access to advanced computers.

"This will mean that any remote laboratory in Asia or Africa where a lot of these recent outbreaks have occurred will be able to use a portable, field-ready benchtop sequencer hooked up to a smartphone or laptop with an Internet connection, to obtain a complete genetic sequence of a novel pathogen within hours," said Chiu. "Our hope is that these efforts will democratize the surveillance and investigation of infectious diseases."

Laird revenue slips as demand from 'largest customer' drops

(Reuters) - Electronic-components maker Laird Plc said revenue from its largest customer -- widely assumed to be Apple Inc -- fell 17 percent in the first quarter from a year earlier, contributing to a 2 percent fall in overall revenue.

Laird, which makes electromagnetic shielding and heat control components for wireless devices, said it expected another quarter of declining revenue before growth resumes in the second half of the year as customers and the company itself launch new products. It maintained its full-year forecast.

Total revenue fell to 119 million pounds ($184.59 million) for the quarter ended March 31 from 122 million pounds a year earlier, the company said in an interim management statement.

A company representative declined to identify the largest customer, but analysts said Laird has confirmed in the past that it is Apple and that they assumed the company was referring to the iPhone and iPad maker in its statement on Friday.

"I think it's become clear that there had been a overbuild through Q4 by the said largest customer and I think therefore as we got into Q1 the general weakness was exacerbated by destocking cycle," Peel Hunt analyst Dominic Convey said.

Convey also noted that lower-cost smartphone makers were taking market share from Apple.

Laird also supplies components to Samsung Electronics Co Ltd.

Revenue fell marginally in both the core performance materials division, which makes devices for smartphones and tablets, and the wireless systems business.

The materials division accounted for 73 million pounds of revenue while the wireless systems business contributed 47 million pounds.

The wireless business has been affected by a slowdown in wireless spending and lower sales to the public safety market, Laird said.

Brokerage Investec put its forecasts, target price and recommendation under review following the results and said it initially expected to trim its forecasts by 2-3 percent.

Investec has "hold" recommendation on the stock, with a target price of 240 pence. Laird's shares were down 3.3 percent at 209.7 pence on the London Stock Exchange at 7:35 a.m. ET.

The company, which also makes smart chips for GPS and radio systems in cars, reported a 17 percent rise in full-year profit in March, citing growing demand in the smart-device market.

North Korea Report: Pentagon Says Pyongyang Still Working Toward Goal Of Striking U.S. With Nukes

WASHINGTON -- North Korea "will move closer" to its announced goal of being able to strike the U.S. with a nuclear-armed missile if it keeps investing in tests of nuclear and missile technology, the Pentagon said Thursday in a report to Congress.

The unclassified version of the report, which was required by a 2012 law, offered no estimate of when North Korea might achieve that capability. It said the pace of progress will depend in part on how many resources are invested.

The report fits an established U.S. intelligence picture of North Korea making an enormous effort to become a nuclear power and of an economically poor country directing a disproportionate amount of resources to its military.

Much about North Korea is a mystery to Western intelligence agencies, including the intentions of its leader, Kim Jong Un, who came to power after his father, Kim Jong Il, died in December 2011. The Pentagon report said the U.S. foresees little change in North Korea's key strategic aims, which it said to include using "coercive diplomacy" to compel acceptance of its security interests, as well as developing a nuclear arsenal and undermining of the U.S.-South Korean alliance.

"We anticipate these strategic goals will be consistent under North Korea's new leader, Kim Jong Un," it said.

U.S. intelligence agencies are not fully in agreement on how far North Korea has advanced in its effort to make a nuclear weapon small enough to fit atop a ballistic missile. In April, a U.S. congressman disclosed that the Defense Intelligence Agency believes with "moderate confidence" that the North could deliver a nuclear weapon by ballistic missile but with "low reliability." The DIA assessment did not mention the potential range of such a strike.

Director of National Intelligence James Clapper, the top U.S. intelligence official, said shortly after the DIA assessment was made public that its conclusion was not shared by other intelligence agencies. Clapper said North Korea has made progress but has not "fully developed, tested or demonstrated the full range of capabilities necessary for a nuclear-armed missile."

In its report Thursday, the Pentagon made no mention of the DIA report.

The Pentagon asserted that North Korea wants to leverage the perception that it poses a nuclear threat in order to counter technologically superior forces. South Korea, which does not have nuclear weapons, has a modern military that benefits greatly from a close alliance with the U.S. There are about 28,500 American troops based in the South.

The Pentagon report noted that North Korea has recently showcased its advances in missile technology, including an April 2012 parading of a new road-mobile intercontinental ballistic missile that the Pentagon says has not been flight tested.

"These advances in ballistic missile delivery systems, coupled with developments in nuclear technology ... are in line with North Korea's stated objective of being able to strike the U.S. homeland," the report said.

After a February 2013 nuclear test, North Korea made what the Pentagon called "authoritative public announcements" of its desire to field nuclear-armed missiles with sufficient range to attack targets in the United States.

"North Korea will move closer to this goal, as well as increase the threat it poses to U.S. forces and allies in the region, if it continues testing and devoting scarce regime resources to these programs," the report said.

Earlier this year, North Korean leader Kim Jong Un made a series of bellicose threats to attack South Korea, Japan or the United States with nuclear weapons, sparking tough rhetoric in return. In response, the Pentagon in April announced plans to beef up its missile defenses by deploying 14 additional missile interceptors at a military base in Alaska.

Thursday's Pentagon report said the North's work on a space-launch vehicle has contributed heavily to its effort to build a missile capable of reaching the U.S. with a nuclear warhead. That work was highlighted by the launch of a satellite into space last December.

But it added that the North has yet to test a re-entry vehicle, without which it cannot deliver a warhead to a target. A workable re-entry vehicle is necessary to get a warhead back into Earth's atmosphere with protection against severe heating.

The report also projected that North Korea under Kim will stick to its current strategic priorities, including developing nuclear weapons to deter any attack from outside powers and trying to undermine the alliance between the United States and South Korea.

Taking cholesterol-lowering drugs may also reduce the risk of dying from prostate cancer

May 1, 2013 ? Men with prostate cancer who take cholesterol-lowering drugs called statins are significantly less likely to die from their cancer than men who don't take such medication, according to study led by researchers at Fred Hutchinson Cancer Research Center. The findings are published online today in The Prostate.

The study, led by Janet L. Stanford, Ph.D., co-director of the Prostate Cancer Research Program and a member of the Hutchinson Center's Public Health Sciences Division, followed about 1,000 Seattle-area prostate cancer patients. Approximately 30 percent of the study participants reported using statin drugs to control their cholesterol. After a mean follow-up of almost eight years, the researchers found that the risk of death from prostate cancer among statin users was 1 percent as compared to 5 percent for nonusers.

"If the results of our study are validated in other patient cohorts with extended follow-up for cause-specific death, an intervention trial of statin drugs in prostate cancer patients may be justified," Stanford said.

"While statin drugs are relatively well tolerated with a low frequency of serious side effects, they cannot be recommended for the prevention of prostate cancer-related death until a preventive effect on mortality from prostate cancer has been demonstrated in a large, randomized, placebo-controlled clinical trial," said first author Milan S. Geybels, M.Sc., formerly a researcher in Stanford's group who is now based at Maastricht University in The Netherlands.

The study is unique in that most prior research of the impact of statin use on prostate cancer outcomes has focused on biochemical recurrence -- a rising PSA level -- and not prostate cancer-specific mortality. "Very few studies of statin use in relation to death from prostate cancer have been conducted, possibly because such analyses require much longer follow-up for the assessment of this prostate cancer outcome," Geybels said. The potential biological explanation behind the association between statin use and decreased mortality from prostate cancer may be related to cholesterol- and non-cholesterol-mediated mechanisms.

? An example of the former: When cholesterol is incorporated into cell membranes, these "cholesterol-rich domains" play a key role in controlling pathways associated with survival of prostate cancer cells.

? An example of the latter: Statin drugs inhibit an essential precursor to cholesterol production called mevalonate. Lower levels of mevalonate may reduce the risk of fatal prostate cancer.

"Prostate cancer is an interesting disease for which secondary prevention, or preventing poor long-term patient outcomes, should be considered because it is the most common cancer among men in developed countries and the second leading cause of cancer-related deaths," Geybels said. "While many prostate cancer patients have indolent, slow-growing tumors, others have aggressive tumors that may recur or progress to a life-threatening disease despite initial therapy with radiation or surgery. Therefore, any compound that could stop or slow the progression of prostate cancer would be beneficial," he said.

The National Cancer Institute, a grant from the Dutch Cancer Society and additional support from Fred Hutchinson Cancer Research Center and the Prostate Cancer Foundation funded the research.

Breast Implants Linked To Cancer Deaths | Lifehacker Australia

Women with cosmetic breast implants who contract breast cancer are 38 per cent more likely to die than their natural-breasted counterparts, a new study has found. This is because the implants create shadows on mammograms that can obscure some breast tissue, which makes it more difficult to detect breast cancer at an early stage.

To ascertain whether breast implants increased the risk of terminal breast cancer, researchers from the Public Health Agency of Canada, et al, collated data from 17 previous studies relating to the disease. They discovered a significant link between terminal cancer and cosmetic breast implants, with the risk of dying rising by more than a third.

[The data] showed reduced survival after breast cancer among women who had implants compared with those who did not. This systematic review suggests that women with cosmetic breast implants have later stage tumors at diagnosis of breast cancer [which] adversely affects the survival of women who are diagnosed.

However, the report notes that its findings should be interpreted with caution as some studies did not adjust for other potential confounding factors (such as age at diagnosis and body mass index) and the overall number of studies used was relatively small. (That sound you just heard was a million strippers breathing a sigh of relief.)

Nonetheless, the authors maintain the accumulating evidence suggests that women with cosmetic breast implants who develop breast cancer have an increased risk of being diagnosed as having non-localized breast tumors more frequently than do women with breast cancer who do not have implants. The report concludes that further investigations are warranted.
